Our Practice
Clearwater Cardiovascular Consultants was founded in 1975 by Donald R. Eubanks, M.D., when Morton Plant Hospital opened its first cardiac catheterization laboratory and established a cardiovascular surgery program. Dr. Eubanks retired in 1998.

In 1999 we merged with Interventional Cardiovascular Associates and changed our name to Clearwater Cardiovascular and Interventional Consultants.

Our practice has grown to become one of the the largest cardiology groups in Pinellas County, offering a full range of cardiology services in our offices, Morton Plant Mease Hospitals, and Largo Medical Center.

Our Specialty
Our physicians specialize in cardiovascular medicine, focusing on the diagnosis and treatment of diseases affecting the heart and the blood vessels.

Since our practice is limited to cardiology, all non-cardiac problems will be referred to your primary care physician. If you do not have an Internist or Family Practice Physician, our office will be happy to provide you with a referral.

We will make every effort to schedule a time suitable for you. Occasionally our physicians have emergencies which may require a change in your appointment or a delay in our office. We appreciate your patience and understanding should this situation occur.

Prior to your first appointment, you will be given forms to complete which provide vital medical history for your physician along with other information to help our office serve you better. We will also attempt to secure copies of your previous cardiac medical records to assist our physicians with your diagnosis and treatment.

Office Hours
Our offices are open from 9 am to 5 pm with our telephones being answered from 9 am to 4 pm. One of our physicians is always on call for any after-hour emergency needs which may arise.

After-Hour Emergencies If, after our normal office hours, you find the need to speak with one of our physicians, please call our answering service at: (727) 449-6735 The service will contact our physician on call.

Prescription Refills It is important to plan ahead and anticipate your medication needs. When your medicine is low, please contact your pharmacist for refills allowing the pharmacist enough time to check with your doctor for any needed authorizations. Please allow a minimum of 48 hours for the refill process.

Financial Policy

Medicare
We are a participating provider with Medicare, which means we will file claims on your behalf. Under the Medicare program, each patient is responsible for a 20% co-payment as well as a Medicare deductible. As a courtesy, unless you request otherwise, we will also file your secondary insurance.|

Managed Care (HMO, PPO)
We participate with most major managed care plans. If your visit or treatment needs to be pre-authorized, please make sure this is completed by your primary care physician or managed care plan prior to your appointment. Otherwise, you may find yourself responsible for the entire cost of your visit or procedure.

Medicaid and Social Services
We participate in Florida Medicaid and Medipass. However, eligibility and authorization must be documented in order for us to file your claims.

Other Plans
Our policy is to collect fees at the time of service unless other arrangements have been made. For your convenience, we accept most major credit cards.

Our Patient Accounting Specialists are located in the Clearwater office on the fourth floor of the Cheek-Powell Heart and Vascular Pavilion.
